The PRO-SAFE BD-P01-1 is a double-sided lockout tagout safety tag designed for energy isolation procedures in industrial, commercial, and trade service environments. Constructed from unrippable vinyl, the tag resists tearing during active lockout conditions. Side 1 reads DO NOT OPERATE EQUIPMENT LOCK-OUT THIS TAG LOCK TO BE REMOVED ONLY BY THE PERSON SHOWN ON BACK, and Side 2 reads EQUIPMENT LOCKED OUT BY DATE. The rectangular tag measures approximately 5.71 inches tall by 3.03 inches wide with a single brass or metal grommet hole measuring 0.38 inches in diameter for attachment to lockout hasps, padlocks, or valve handles. Sold in packs of 10.

Installation Notes

Lockout tagout tags are applied by qualified personnel following an established LOTO procedure in accordance with OSHA 29 CFR 1910.147 energy control standards. Before applying the tag, de-energize, depressurize, or otherwise isolate the equipment per site-specific procedures. Thread the tag through a hasp or directly over a lockout point before applying the lock. Tags must not be removed by anyone other than the authorized person identified on the back of the tag.
