The LEVITON MFG CO R66-T5225-0TS is a commercial-grade, tamper-resistant switch and outlet combination device built for use in commercial and institutional wiring applications. The receptacle side features an internal shutter mechanism that blocks contact access unless a two-prong plug is inserted simultaneously into both slots, preventing foreign objects such as hairpins or keys from making contact with live terminals. The switch side is a single-pole configuration. The device is side-wired and accepts up to No. 12 copper wire, with a steel mounting strap for secure installation. The thermoplastic body is rated for standard 15-amp, 125-volt service on a NEMA 5-15R receptacle.
This combination device is suited for commercial, institutional, and light industrial installations where tamper resistance is required by code. Typical applications include schools, healthcare facilities, hotels, and any occupancy where NEC tamper-resistant receptacle requirements apply. The single-pole switch and outlet share a standard single-gang form factor, making it a practical space-saving solution in areas where both switching and outlet access are needed at the same location. This unit is sold in a package of six.
Designed as a commercial-grade replacement or new-installation switch and outlet combination for standard single-gang electrical boxes wired with up to No. 12 copper conductors on a 15-amp, 125-volt circuit.
Installation should be performed by a licensed electrician in accordance with the National Electrical Code (NEC) and all applicable local codes. Always shut off the circuit breaker and verify the circuit is de-energized with a voltage tester before making any wiring connections. Side-wire connections must be properly torqued to terminal screws per the device labeling. Ensure the steel strap is flush and secure in the outlet box before installing the cover plate.
