The ACCUFORMNMC RPT1A is a double-sided accident prevention tag designed for lockout/tagout and equipment isolation applications. Constructed from unrippable vinyl, the tag measures 3 inches high by 6 inches wide and carries a bold DANGER header with black legend text on both sides. Side 1 displays "DANGER - DO NOT OPERATE" with signature and date fields for the authorizing technician. Side 2 reinforces the warning with a full discharge-consequence statement.
