The ACCUFORMNMC RPT14 is a double-sided accident prevention tag designed for lockout/tagout and electrical hazard warning applications. Printed on unrippable vinyl, this rectangular tag measures 3 inches high by 6 inches wide and carries a DANGER header in black legend on both sides. Side 1 reads DO NOT THROW SWITCH MEN AT WORK ON CIRCUIT with signature and date fields. Side 2 reads DANGER DO NOT REMOVE THIS TAG TO DO SO WITHOUT AUTHORITY WILL MEAN POSSIBLE DISCHARGE IT IS HERE FOR A PURPOSE SEE OTHER SIDE. Each pack contains 25 tags. Electricians and safety personnel use these tags to communicate live-circuit hazards and restrict unauthorized switch operation.
