The ACCUFORMNMC LOTAG13-25 is a double-sided lockout danger tag designed for equipment tag-out procedures in industrial, commercial, and trade service environments. Constructed from unrippable vinyl, this tag measures 3 inches high by 6 inches wide and carries a bold DANGER header on both sides. The front legend reads DANGER - DO NOT OPERATE - EQUIPMENT TAG-OUT with fields for a signature and date. The reverse reads DANGER - DO NOT REMOVE THIS TAG. A single brass-style grommet with a 0.38-inch hole diameter allows secure attachment to lockout points. Each pack contains 25 tags.

This tag is used during lockout/tagout (LOTO) procedures required by OSHA 29 CFR 1910.147 before servicing electrical panels, HVAC equipment, pumps, motors, and other hazardous energy sources.
