The ACCUFORMNMC LOTAG11 is a double-sided lockout tag designed for lockout/tagout (LOTO) procedures in industrial and commercial facilities. Made from unrippable vinyl, the tag carries a DANGER header on both sides and printed legends directing personnel not to operate locked-out equipment. Side 1 reads: DANGER - DO NOT OPERATE - EQUIPMENT LOCKED OUT - THIS TAG AND LOCK TO BE REMOVED ONLY BY THE PERSON SHOWN ON BACK. Side 2 provides fields for the equipment locked out by and date. The tag measures 3 inches high by 6 inches wide and includes a single grommet for secure attachment. Sold in a pack of 10.

Installation Notes

Lockout tags are applied by qualified personnel as part of an authorized energy control procedure. The tag should be attached through the grommet directly to the energy-isolating device or lockout hasp using a nylon cable tie, lock, or approved attachment device. Tags should never be used as the sole means of energy isolation without an accompanying lock where lock attachment is practicable.
