The ACCUFORMNMC LLT3 is a double-sided lockout danger tag constructed from unrippable vinyl for use in energy control and lockout/tagout (LOTO) programs. Measuring 3 inches high by 6 inches wide, the tag carries a bold DANGER - DO NOT OPERATE message on the front and a DANGER - DO NOT REMOVE THIS TAG warning on the reverse. Black legend on vinyl provides high contrast readability in industrial and commercial environments. Each tag includes a nylon fastener and one brass grommet for secure attachment to equipment, valves, breakers, and other energy-isolation points. This pack contains 25 tags.

This tag is designed for use by licensed electricians, qualified HVAC technicians, plumbers, and any trade professional required to follow OSHA 29 CFR 1910.147 lockout/tagout procedures. Installation Notes

Lockout/tagout tags must be applied by authorized personnel following a written energy-control procedure in accordance with OSHA 29 CFR 1910.147. De-energize and physically isolate the equipment before attaching the tag. Thread the included nylon fastener through the grommet and secure it to the lockout point or hasp. Tags must not be removed by anyone other than the authorized employee who applied them.
