The ACCUFORMNMC IST4205 is a single-sided energy isolation tag printed on unrippable vinyl and designed for use in lockout/tagout (LOTO) procedures across industrial, mechanical, and facilities environments. Measuring 4 inches high by 2 inches wide, each tag features the front legend "V-01 THRU V-10 DO NOT REMOVE THIS TAG VALVE ISOLATION POINT" with a blank back for field notation. The durable vinyl construction resists tearing, making it suitable for demanding service conditions. Each pack contains 10 tags. These tags are applied by maintenance technicians, electricians, pipefitters, and safety personnel during equipment isolation workflows.

Installation Notes

Energy isolation tags are applied by qualified personnel following a written lockout/tagout procedure in compliance with applicable OSHA 29 CFR 1910.147 energy control standards. Power down, de-energize, or isolate the relevant valve or equipment before attaching the tag. Thread the grommet attachment point through a hasp, lock, or valve handle as required by the site energy control procedure. Tags must not be removed except by the authorized employee who applied them or as specified in the site LOTO program.
