The PRO-SAFE BD-P01-4 is a double-sided lockout tagout safety tag designed for energy isolation procedures in industrial, commercial, and facility maintenance environments. Constructed from unrippable vinyl, this tag holds up in demanding jobsite conditions where paper tags fail. Side 1 displays the legend DO NOT OPERATE EQUIPMENT LOCK-OUT THIS TAG LOCK TO BE REMOVED ONLY BY THE PERSON SHOWN ON BACK, and Side 2 reads EQUIPMENT LOCKED OUT BY DATE. A single brass or zinc grommet reinforces the hang hole for secure attachment to lockout points, valves, breakers, and disconnect handles. Installation Notes

Lockout tagout tags are applied by the authorized employee performing the service, after all energy sources have been isolated and de-energized per the site-specific energy control procedure. Tags should be attached directly to the energy-isolating device using the grommet hang hole, and must not be removed by anyone other than the person who applied them. Consult OSHA 29 CFR 1910.147 for full lockout tagout procedural requirements. Do not substitute tags for physical lockout devices where locks are required by procedure.
