Side one features 15 rows with two in/out boxes per row, plus fields for name, ID number, period description, signature, and totals for regular time, overtime 1, overtime 2, and gross time. Side two supports semi-monthly and monthly periods with 16 rows, each containing two in/out boxes, along with fields for notes, name, ID number, and signature. Cards are sold in a pack of 100.
