The ACCUFORMNMC TAR416 is a rectangular lockout tag designed for lockout/tagout (LOTO) safety programs in industrial, commercial, and trade environments. Printed on durable synthetic paper that resists cracking, distortion, and shrinking, each tag features a bold DANGER header with the legend Locked Out Do Not Operate printed in black on a white background. The tag measures 3 inches wide by 6.25 inches high and is double-sided with two printed faces, ensuring the warning is visible from either direction. Tags are connected end-to-end with perforations for easy separation and are housed in an octagon-shaped cardboard dispenser box for organized jobsite or panel room storage.
This lockout tag is used in residential and commercial service environments wherever equipment must be isolated and secured before maintenance or repair. Electricians, HVAC technicians, plumbers, and facility maintenance personnel use LOTO tags to communicate equipment status during service calls and scheduled shutdowns. The synthetic cardstock construction holds up in damp mechanical rooms and utility spaces where standard paper tags would degrade. Tags are printed in English and meet OSHA and ANSI standards, making them appropriate for regulated worksites.
Designed as a standard LOTO danger tag for use with lockout hasps, padlock setups, and energy-isolation points on electrical panels, valves, and mechanical equipment in any trade or industrial setting.
Lockout tags are installed by licensed electricians, HVAC technicians, plumbers, or qualified maintenance personnel as part of a formal lockout/tagout procedure. All energy sources must be fully isolated and de-energized before tagging equipment. Tags should be attached through the 0.375-inch hole using an appropriate tie, nylon strap, or cable lockout device. Consult OSHA 29 CFR 1910.147 and applicable site-specific LOTO programs before beginning any service procedure.
