The ACCUFORMNMC RPT30 is a double-sided accident prevention tag designed for hazard identification and lockout/tagout applications in industrial, commercial, and trade environments. Constructed from unrippable vinyl, this rectangular tag measures 3 inches high by 6 inches wide and carries a bold DANGER header in black and red. Side 1 displays a DANGER - SIGNED BY ___ legend with fields for signature and date. Side 2 carries a DANGER - BARRICADE TAG legend with a full-text warning against unauthorized tag removal.
