The ACCUFORMNMC LPT21 is a DANGER lockout tag made from unrippable vinyl for use in lockout/tagout (LOTO) safety procedures. Measuring 3 inches high by 6 inches long, the tag is printed on both sides with a bold DANGER header. The front legend reads DANGER - DO NOT OPERATE - THIS LOCK/TAG MAY ONLY BE REMOVED BY: NAME, DEPT, EXPECTED COMPLETION, providing write-in fields for the authorized technician. The back legend reads DANGER - THIS ENERGY SOURCE HAS BEEN LOCKED OUT. Each pack includes nylon fasteners for attachment. This tag is used by electricians, HVAC technicians, plumbers, and other trades performing energy isolation work.

The LPT21 is suited for industrial, commercial, and residential service environments where OSHA-compliant lockout/tagout procedures are required. The unrippable vinyl construction holds up in demanding conditions including exposure to moisture, heat, and mechanical stress. Installation Notes

Lockout tags are applied by the authorized servicing technician after all energy sources have been isolated and locked out in accordance with OSHA 29 CFR 1910.147 control of hazardous energy procedures. Complete all write-in fields on the front of the tag before attaching. Secure the tag to the lockout point using the included nylon fastener. The tag must only be removed by the individual named on the front face upon verified completion of the work.
