The ACCUFORMNMC LOTAG8-25 is a lockout danger tag made from unrippable vinyl, designed for use in energy control and lockout/tagout (LOTO) procedures. Each tag measures 6 inches in length by 3 inches in height and features a bold DANGER header on a two-sided printed face. The front legend reads DANGER - DO NOT ENERGIZE - EQUIPMENT LOCK-OUT - THIS TAG AND LOCK TO BE REMOVED ONLY BY THE PERSON SHOWN ON BACK, while the back provides space to identify the authorized employee. A single brass or metal grommet allows the tag to be secured to equipment with a lockout hasp or cable. This pack contains 25 tags.

Installation Notes

Lockout/tagout tags must be applied by a qualified, authorized employee in accordance with the facility energy control program and OSHA 29 CFR 1910.147. Power down, de-energize, and verify zero-energy state before attaching the tag. Tags must be used in conjunction with a physical lockout device and may only be removed by the person identified on the back of the tag. Do not reuse tags that show signs of damage or altered markings.
