The ACCUFORMNMC LOTAG37-25 is a double-sided lockout danger tag designed for use in lockout/tagout (LOTO) safety programs. Constructed from unrippable vinyl, this tag measures 6 inches wide by 3 inches high and carries a bold Danger header with a black legend on both sides. Side 1 reads: DANGER - EQUIPMENT LOCKED OUT - THIS LOCK/TAG MAY ONLY BE REMOVED BY, with fields for name, department, and expected completion. Side 2 reads: THIS ENERGY SOURCE HAS BEEN LOCKED OUT - UNAUTHORIZED REMOVAL OF THIS LOCK/TAG MAY RESULT IMMEDIATE DISCHARGE, with a remarks field. Each pack contains 25 tags. Lockout/tagout tags must be applied by qualified personnel following OSHA 29 CFR 1910.147 energy control procedures. Always de-energize, isolate, and verify zero energy state before attaching the tag. Tags should be used in conjunction with an approved lockout device and must not be removed by anyone other than the authorized employee who applied them. No tools are required for tag application, but a compatible lockout hasp or device is typically used alongside these tags.
