The ACCUFORMNMC LOTAG34 is a double-sided lockout danger tag designed for lockout/tagout (LOTO) safety programs in industrial, commercial, and mechanical service environments. Constructed from unrippable vinyl, this tag measures 3 inches high by 6 inches wide and features a single brass or steel grommet for secure attachment to energy isolation points. The front legend reads LOCKED OUT - DO NOT OPERATE with supporting LOTO compliance text. The reverse side provides fields for equipment identification, technician name, and date. Each pack contains 10 tags.

Installation Notes
