The ACCUFORMNMC LOTAG17 is a double-sided lockout danger tag designed for lockout/tagout (LOTO) procedures in industrial, commercial, and trade service environments. Constructed from unrippable vinyl, the tag measures 3 inches high by 6 inches wide and carries a bold DANGER header printed in black on both sides. Side 1 reads: DANGER - EQUIPMENT LOCKED OUT - THIS TAG AND LOCK TO BE REMOVED ONLY BY THE PERSON SHOWN ON BACK. Side 2 reads: DANGER - EQUIPMENT LOCKED OUT BY with a date field. A single brass or metal grommet with a 0.38-inch hole diameter allows attachment to lockout hasps, padlocks, or energy isolation points. Sold in packs of 10.

Installation Notes
