Product Description

The ACCUFORMNMC JMTAG2 is a self-laminated vinyl lockout tag designed for lockout/tagout (LOTO) safety procedures across industrial, mechanical, and electrical service environments. The tag carries a bold DANGER header and features double-sided printing with a photo identification field on the front face, reinforcing personal accountability during equipment isolation. Measuring 4 inches high by 7.38 inches wide, the rectangular tag includes a single grommet with a 0.38-inch hole diameter for attachment to lockout hasps, valves, circuit breakers, and other energy-isolation points. The self-laminating vinyl construction protects printed and handwritten information from moisture, abrasion, and shop-floor conditions. Tags are sold in packs of 10.

This lockout tag is used wherever a written LOTO procedure requires a physical tag to identify the authorized person who applied the lock. Common applications include electrical panel lockout by licensed electricians, valve and pipe isolation by plumbers and pipefitters, HVAC unit and refrigeration equipment servicing, and general mechanical lockout on production or facility maintenance equipment. Installation Notes

Lockout tags must be applied by a qualified person in accordance with OSHA 29 CFR 1910.147 energy control procedures and any applicable facility-specific LOTO program. De-energize, isolate, and verify zero energy state before applying the tag and lock. Complete all handwritten fields on the tag before attachment. Tags must not be removed except by the authorized person whose identification appears on the front face.
