The ACCUFORMNMC CU-307217 is a lockout danger tag designed for use in energy-control and lockout/tagout (LOTO) procedures across industrial, commercial, and trade environments. Printed on unrippable vinyl for durability in demanding conditions, each tag measures 7 inches high by 4 inches wide and features a bold orange-on-orange layout with a single grommet for secure attachment to lockout points. The front legend reads: LOCKED OUT - DANGER - EMPLOYEE NAME, COMPANY NAME, ID#, PHONE#, giving technicians a clear, fill-in field format to identify who has locked out a piece of equipment.

These tags are used during electrical, mechanical, pneumatic, and hydraulic energy isolation procedures. Installation Notes

Lockout tags must be applied only after the energy-isolating device has been locked in the safe position per OSHA 1910.147 and applicable facility LOTO programs. Always pair tags with an approved lockout device - tags alone do not constitute a compliant lockout when a lockout device can be applied. Removal of a lockout tag must be performed only by the authorized employee who attached it.
